Form building is typically managed internally and limited to a selected few users in a compay's tenant. If you are unsure about the responsibility of this task, please contact your company's admin user(s).
There are two ways to build a form:
1. Manually build forms within the form building tool
2. Import from Excel
The differences in each process are:
- Question type determines the answer style and options prompted to complete your scope of work
- Answer style may vary depending on your customer's requirements and/or any internal standards set by your team
Note: Currently, when importing from Excel, the default answer options for each question will be Pass and Fail with the radio button style.
You can add additional, answer options or modify those answer options in the form building tool or manually build each question with different answer options. There is no limit to the number of checklist items, question types or answer styles that can be added when building a form.
Getting Started
Below is an introduction to the standard answer styles. Remember that these items are specific to the form-building process and cannot be modified while actively working in a session. We recommend reviewing the chart below to determine the needs for your scope of work.

Question Type | Answer Style | About | Example |
Tabs | * | Rather than a specific answer style like the others below, tabs allow for you to add additional positions or sectors for the same set of questions. For example, if you have multiple items or equipment that require the same set of questions, you can add additional tabs to provide the required photos or images for each. | |
Textbox | Open text field | Textbox requires an open, text field answer rather than a pre-filled list of options to select while completing your scope of work. We recommend this for short lines of text. | |
Textarea | Open text field | Textarea functions the same as Textbox but allows for additional characters and we recommend this option for multiple lines of the required information. | |
Checkbox List | Muti-select | Checkbox list can be a set of pre-filled options that users can select multiple answer options. | |
Radiobuttons | Single-select | Radiobuttons can also be a set of pre-filled options, but users can only select one option. | |
Dropdown | Single-select | Dropdown is another set of pre-filled options that allows users to select one option. Some prefer this with a list of options exceeding 5 or more answer options. | |
Date | Single-select | Date will generate a popup calendar for users to quickly select the date and time. |
There is a 1:1 ratio for associating images or videos to a single checklist item within a session. In other words, you can capture as many items as you need in the session's gallery, but you can only assign one photo or video to a single question. Our recommendations below:
Option 1 For a checklist item that requires multiple images, we recommend a short video to associate with that form question/ requirement.
Option 2 You can also break down the checklist into as many requirements as you need. For example, if one checklist item requires 6 photos, you can add 6 questions that each ask for a required photo. While this may seem like additional work, this simply ensures that all required items are covered in your scope of work.

Lastly, once a form has been completed and ready for use, you will then need to publish the form so that other users can access and utilize for their scope of work as well.
